Vaccines at At birth: Hepatitis B (HB-0)
Before your baby leaves the hospital or birth facility, one vaccine should already be given: Hepatitis B (HB-0). It is the first step of the complete primary immunization series and is given on day one of life.

Hepatitis B (HB-0)
- Protects against
- Hepatitis B infection, including mother-to-baby transmission at birth that can lead to long-term liver disease.
- How it is given
- Injection into the thigh (intramuscular).
- Side effects
- Mild fever and swelling or soreness at the injection site for 1–2 days.
- Availability
- Free at every birth facility (hospital, puskesmas, maternity clinic, midwife) and at posyandu.
- Note
- Should be given within the first 24 hours after birth. Babies born under 2,000 g repeat the dose at 1 month (IDAI recommendation).
Why it matters at this age
The hepatitis B virus can pass from mother to baby during delivery. Infected infants face a high risk of becoming lifelong carriers, with later risks of cirrhosis and liver cancer. Giving HB-0 within the first 24 hours cuts this transmission route early.
What to bring to posyandu
HB-0 is given at the birth facility before discharge. Make sure it is recorded in the KIA (Maternal and Child Health) book. For home births, ask your midwife to give HB-0, or take the baby to the nearest puskesmas or posyandu promptly. Always bring the KIA book to every visit.
Frequently asked questions
My baby did not get HB-0 at the birth facility. Is it too late?
Not too late. Take your baby to the nearest puskesmas or posyandu. HB-0 is still beneficial within the first weeks of life. Discuss the follow-up schedule with your midwife or doctor.
Is HB-0 safe for premature babies?
Yes, it is safe and even more important, since premature babies are more vulnerable. Birth weight is not a reason to delay. If birth weight was under 2,000 g, IDAI recommends repeating the dose once the baby reaches 1 month of age.
